The Suitability between the Supply and Use of Blood Transfusion in Major Gynecological Operations at the Adam Malik Hajj Center General Hospital Medan

Gynecological surgery, especially major surgery, has a high risk of bleeding that can threaten the patient's life. Blood transfusion is an important solution in handling massive bleeding during surgery. However, the Pandemi Covid-19 presents new challenges in the supply of blood. The decrease in the number of blood donors and uncertainty of intraoperative blood needs result in an increase in unused blood products. This study aims to analyze the suitability between the supply and use of blood transfusion in major gynecological operations at the Adam Malik Hajj General Hospital in Medan.

Gynecological surgery is a common procedure performed in hospitals worldwide. However, major gynecological surgery has a high risk of bleeding, which can lead to serious complications and even death. Blood transfusion is a critical intervention in managing massive bleeding during surgery. The availability of blood products is essential to ensure the success of blood transfusion. However, the Pandemi Covid-19 has presented new challenges in the supply of blood. The decrease in the number of blood donors and uncertainty of intraoperative blood needs have resulted in an increase in unused blood products.

This study was a latitude cutting study that aimed to analyze the suitability between the supply and use of blood transfusion in major gynecological operations at the Adam Malik Hajj General Hospital in Medan. The study was conducted by analyzing the patient's medical record data undergoing major gynecological surgery between March 2019 and March 2021. Data was collected from 255 samples, with 215 samples from the pre-Pandemic period and 40 samples from the Pandemic period.

The results showed a significant compatibility between the supply and use of blood transfusion. The average blood supply reaches 807cc, while the average blood use is 617cc, with a p value <0.001. This finding indicates that the hospital succeeded in meeting the patient's blood needs during major gynecological surgery, even in the middle of Pandemic.

This study provides important information about the blood management blood management strategy in hospitals, especially in dealing with emergency situations such as Pandemic. The suitability between the supply and use of blood transfusion shows that the blood management system in the hospital has been effective in meeting the needs of patients. However, this research has several limitations. This study is only conducted in one hospital, so the generalization of research results needs to be carefully considered. In addition, this research does not include information about the type of gynecological operation carried out, which can affect the blood needs of transfusion.

In conclusion, this study has shown that the Adam Malik Hajj General Hospital in Medan has been successful in meeting the patient's blood needs during major gynecological surgery, even in the middle of Pandemic. However, further research with broader scope and more detailed data regarding the type of gynecological operation, risk factors for bleeding, and the needs of individual transfusion blood, needed to maximize the effectiveness of transfusion blood management in hospitals and ensure the availability of blood for patients in need.

Based on the findings of this study, the following recommendations are made:

Blood management strategy: The hospital should continue to implement its blood management strategy, which has been effective in meeting the needs of patients.
Blood supply: The hospital should ensure that there is a sufficient supply of blood products to meet the needs of patients.
Blood use: The hospital should ensure that blood products are used efficiently and effectively to minimize waste.
Further research: Further research should be conducted to investigate the effectiveness of blood management strategies in other hospitals and to identify areas for improvement.

This study has several limitations. This study is only conducted in one hospital, so the generalization of research results needs to be carefully considered. In addition, this research does not include information about the type of gynecological operation carried out, which can affect the blood needs of transfusion.
