Introduction

Iron deficiency anemia is a significant public health problem in many parts of the world, including Indonesia. In Aceh Besar Regency, the distribution of iron tablets (Fe) is very low, with only 12.40% of pregnant women consuming Fe1 tablets and 10.56% consuming Fe3 tablets. This low distribution can potentially increase the prevalence of anemia in pregnant women, which can have serious consequences for both the mother and the child. However, there is a lack of concrete data regarding the level of anemia or compliance with pregnant women in consuming Fe tablets. One of the main causes of this problem is the weak system of monitoring, recording, and reporting carried out by health workers related to the consumption of Fe tablets and examining hemoglobin (HB) levels routinely.

The Role of Health Workers in Improving Compliance

Health workers play a crucial role in improving compliance with pregnant women in consuming Fe tablets. They can act as customers, communicators, motivators, facilitators, and counselors to encourage pregnant women to consume Fe tablets regularly. The type of research used in this study is prospective cohort research, with the population including all pregnant women who have made health visits at the local health center. Data collection methods are carried out through interviews and health checks, followed by data processing that includes editing, coding, entry, and cleaning. Data analysis is carried out in a univariate, bivariate, and multivariate.

The Effect of the Role of Health Workers on Compliance

The results of this study showed that the variables that had a significant influence on the compliance of pregnant women in consuming Fe tablets were the role of health workers as customers and facilitators. The customer's role variable is proven to have the greatest influence with an odds ratio (OR) value of 0.242. This shows that pregnant women who accept the role of health workers as customers well have the possibility of 0.242 times greater to obey in consuming Fe tablets compared to pregnant women who do not accept that role.

Recommendations

Based on the results of this study, it is recommended that the Puskesmas expand the distribution of Fe tablets for free according to government programs, which is 30 tablets per visit or a total of 90 tablets during pregnancy. In addition, it is important to increase the role of health workers in monitoring, recording, and reporting compliance with pregnant women in consuming Fe tablets. By increasing the support of health workers, it is expected to reduce the prevalence of anemia in pregnant women and improve the health of mothers and children in Aceh Besar Regency.

Conclusion

The importance of the active role of health workers in facilitating and motivating pregnant women to consume Fe tablets cannot be underestimated. Appropriate interventions and support from health workers are expected to increase compliance with pregnant women, so that overall public health can be maintained. Given the high risk of anemia in pregnant women, the strategic steps taken can be a key to improving the health of mothers and children in Aceh Besar Regency.

The findings of this study have significant implications for public health policy in Aceh Besar Regency. The results suggest that increasing the role of health workers in monitoring, recording, and reporting compliance with pregnant women in consuming Fe tablets can lead to improved compliance and reduced prevalence of anemia. This can be achieved by providing training and support to health workers to enhance their skills and knowledge in counseling and motivating pregnant women to consume Fe tablets. Additionally, the government can consider expanding the distribution of Fe tablets for free according to government programs, which can increase access to this essential medication for pregnant women.

This study has several limitations that should be noted. Firstly, the study was conducted in a specific region, Aceh Besar Regency, and the findings may not be generalizable to other regions. Secondly, the study relied on self-reported data from pregnant women, which may be subject to bias and error. Finally, the study did not control for other factors that may influence compliance, such as socioeconomic status and education level.
